Temporary Total Disability Deferment Request (Borrowers
with at least one outstanding loan made BEFORE 7-1-93)
To be used for:
- Deferment based on Stafford, SLS, PLUS, or Consolidation
borrower's, spouse's, or dependent's temporary total disability.

Education Related Deferment Request
To be used for either:
- Deferment while Stafford, SLS, PLUS, or Consolidation
borrowers complete their graduate fellowship.
- Deferment while Stafford, SLS, PLUS, or Consolidation
borrowers receive rehabilitation training.
- Deferment while Stafford, SLS, or PLUS borrowers complete
their internship or residency.
- Deferment while Stafford, or SLS borrowers teach in a
designated teacher shortage area.

Economic Hardship Deferment Request (Borrowers with all
outstanding loans made ON/AFTER 7-1- 93).
To be used for:
- Deferment while Stafford, SLS, PLUS, or Consolidation
borrowers are experiencing economic hardship.

Parental Leave/Working Mother Deferment Request (Borrowers
with at least one outstanding loan made BEFORE 7-1-93).
To be used for either:
- Deferment for Stafford or SLS borrowers who are working
mothers who meet income eligibility requirements.
- Deferment for Stafford or SLS borrowers on parental leave.

Public Service Deferment Request (Borrowers with at least
one outstanding loan made BEFORE 7-1-93).
To be used for either:
- Deferment for Stafford, SLS, or PLUS borrowers on
full-time active duty in the Armed Forces or National Oceanic and
Atmospheric Administration, or as Commissioned Officers in the
Public Health Service.
- Deferment for volunteer Stafford, SLS, or PLUS borrowers
in the Peace Corps, ACTION program or tax exempt organization.

In-School Deferment Request
To be used for either:
- Deferment for Stafford, SLS, PLUS, or consolidation
borrowers enrolled in school full-time.
- Deferment for Stafford, SLS, PLUS, or consolidation
borrowers enrolled half-time but did not have an outstanding
balance on a loan made before 7-1-87.

Unemployment Deferment Request (All borrowers).
To be used for:
- Deferment for unemployed Stafford, SLS, PLUS, or
Consolidation borrowers - 24 or 36 month limit.

Military Deferment Request
To be used for either:
- Deferment for Stafford, SLS, PLUS, or Consolidation
borrowers who are serving on active duty during a war or other
military operation or national emergency.
- Borrowers who are performing qualifying National Guard
duty during a war or other military operation or national
emergency.

Income Based Repayment (IBR) Forms
This repayment plan was developed
to assist borrowers who are having difficulty making their
payments because they have high student debt levels relative to
their incomes. The program is available for Stafford Loans,
PLUS Loans to graduate students and Consolidation Loans with
no underlying parent PLUS Loans. Borrowers must exhibit a
partial financial hardship (PFH) as defined by federal
regulations to enroll in an IBR plan. PFH is based upon the
borrower's income, poverty level for the borrower's state of
residence, family size and student loan indebtedness.
Borrowers must recertify family size and income each year.
This plan became available July 1, 2009. For more information

Form
